An in depth assessment is key to the starting point from which we continue to put together a case management plan and set client focused goals.

This plan considers the needs of the client and what will enable them.  Be that specialist rehabilitation and therapy, care and support.  The report provides costings for the recommendations and timescales.

We can also provide Initial Needs Assessments and Code of Rehabilitation reports

All of our case managers have a clinical qualification and are experienced in working with clients with brain injury. Our case managers cover a wide geographical area working a far as Sheffield and down to Wales and also cover the Midlands.

We work closely as a team and co-case manage to provide continuity for the client as holiday sickness and the unexpected are a fact of life.

We facilitate the care package tailored to each individual client and their situation; every one of our clients is unique and there is no formula to work to. What makes this work is the skill of the case manager and their knowledge of the client and how their brain injury has impacted on them.

We provide training for support workers about brain injury and how it has affected the individual client. We also provide supervision and support-to-support workers, which is essential to maintain a consistent team and ensure that rehabilitation plans in place are implemented and the client is progressing working towards their set goals.  

We can also provide bespoke training for external agencies on brain injury .

We undertake manual handling risk assessments and can provide Manual Handling Training to our clients, as well as external agencies if required. Our background enables us to take into consideration the complex needs of this client group.

We have forged good communication links with a wide variety of professionals and work closely with them to provide the therapeutic interventions recommended.
